• Medicare Advantage - The Medicare Advantage Plans in the state of Texas are non-standardized, which means the benefits will be based on the county you live in. Under Medicare Advantage Plan, Medicare Part A and B will not be your primary insurance, which means you can only visit certain doctors and hospitals. You may choose from three types of provider networks:

• HMO (Health Maintenance Organization) Plan - this plan will enable you to see doctors and other healthcare professionals who participate in your preferred insurance provider’s network. If your doctor is already included in the network, this is a good option because you end up paying less out-of-pocket with doctors within the network.

• PPO (Preferred Provider Organization) Plan - this plan will cover both in and out-of-network healthcare providers, which gives you the freedom and flexibility to choose any doctor who accepts Medicare assignments.

• PFFS (Private-Free-For-Service) Plan - this plan identifies how much it will pay providers and how much you need to pay out-of-pocket when you receive care. The attending doctor has to agree with the plan’s payment terms and agree to provide you treatment. In the case that a doctor disagrees to these terms, the PFFS plan will not cover the services provided by that doctor. 

• Medicare Supplement - these are plans that provide Medicare Plans A and B beneficiaries with additional coverage. Also known as Medigap Insurance policy, this plan requires you to pay your preferred insurance carrier extra monthly premium. In return, the carrier will then pay your Medicare deductible payments, copayments, and co-insurance. Plans are identified by letters and offer the same type of coverage across all insurance carriers. The cost and out-of-pocket expenses will carry from company to company, and in some cases even location to location within the state of Texas. 

• Medicare Prescription Drug - also called, Medicare Part D, this plan is provided by insurance carriers and are regulated solely by Medicare. Unlike Medicare Supplement or Medigap, Medicare Prescription Drug plan between carriers feature different benefits. The monthly premium, formulary, and prior authorizations may vary between Medicare Prescription Drug plans across carriers. In Texas, you can choose from these two options (but not both) for obtaining prescription drug coverage through Medicare Prescription Drug Plan:

• You may sign up for a Medicare Advantage Prescription Drug plan that combines medical and prescription drug benefits into a single plan. This is accessible for policyholders with Medicare Parts A and B. 

• You may enroll in a standalone Medicare Part D Prescription Drug plan that will work alongside Medicare Part A and/or Medicare Parts A and B coverage. 

• Both of these Medicare Prescription Drug Plan have pharmacy networks that help reduce prescription drug costs. Both plans enjoy the following benefits:

• List of formularies or medications that are covered by the plan. This list includes commonly prescribed brand-named medications and less expensive generic counterparts.

• Needs copayments, coinsurance amounts, and annual deductible that are paid for prescription medications covered by the plan. 

• Presence of networks of contracted pharmacies in the state of Texas. 

• Requires a policyholder to live within a plan’s serviceable area.
